Initial Online Consultation

This is a booking for an initial online consultation for all treatments available in clinic.  Preceding the online consultation you will be asked to fill out an online medical information form. This form can be completed online and does not require a printer or scanner.

Cancellation Policy: A £50 booking fee secures your booking at your preferred date and time. Booking fees are non refundable but redeemable against treatment on the day of your treatment. If you are found to be unsuitable for treatment, then the booking fee will be refunded. If you fail to arrive for your scheduled appointments or do not give 48 hours notice then your booking fee will be lost. If you need to change your appointment please contact us one week in advance of your booking to change the appointment time. The booking fee will be refunded once you have attended for your review. Failure to attend your review without notice will result in you losing the booking fee. Please notify us if you need to postpone your review appointment. Please note review appointments need to be completed 3 weeks after treatment is completed.
